Dental Admission Test DAT

The Dental Admission Test (DAT) is a multiple-choice standardized exam taken by potential dental school students in the United States and Canada (although there is a separate Canadian version with differing sections, both American and Canadian versions are usually interchangeably accepted in both countries' dental schools. This article will specifically describe the American DAT). The DAT is a computer based test that can be administered almost any day of the year. Tests are taken at Thomson Prometric testing centers throughout the United States after the preliminary application through the American Dental Association is completed. Each applicant may only take the test a total of three times before having to ask special permission to take the exam again. After taking the exam you must wait 90 days before repeating it. Each exam costs $205.



Test Sections

The DAT comprises four sections: survey of the natural sciences (90 minutes), perceptual ability (often called the PAT, 60 minutes), reading comprehension (60 minutes), and quantitative reasoning (45 minutes). The mathematics of the quantitative exam is similar to that of the SAT. The first section is divided into questions about biology (40 questions), general chemistry (30 questions), and organic chemistry (30 questions). The second section is divided into six different problems sets designed to test perceptual ability, specifically in the areas of three dimensional manipulation and spatial reasoning. The third section of the DAT is divided into three academic essays, each of which is followed by questions about the passage's content. The final section tests basic mathematics skills, with emphasis placed on algebra, critical thinking, fractions, roots, and trigonometric identities.

Immediately after completion of the test, eight standard scores on a scale of 1-30 are calculated and passed on to the test taker. The first six scores come directly from the test: perceptual ability, reading comprehension, quantitative reasoning, biology, general chemistry, and organic chemistry. The remaining two scores reported are summaries of the previous six: the Academic Average is the average of five scores rounded to the nearest whole number: quantitative reasoning, reading comprehension, biology, general chemistry, and organic chemistry. The Total Science score is a standard score based on all 100 questions in the biology, general chemistry, and organic chemistry tests. Dental schools frequently summarize their applicant's scores by listing the academic, science, and perceptual ability (PAT) scores they typically see in their matriculating classes. Schools tend to emphasize the importance of high perceptual ability and biology scores, though the competitive applicant earns high marks across all sections of the test.
The mean (average) score for any scored section is set at 17. Scores above and below this represent fractions of standard deviations from the mean. This probabilistic scoring system results in the maximum not occurring for the compiled section scores (natural sciences and academic average) in a given year. For example, in 2003 a 25 academic average was labeled as 100.0th percentile, such that less than 8 people received this score, and none higher (approximately 8,000 people take the DAT per year).
The mean academic average score for admissions is commonly 18.[1] A score of 20-22 will likely lead to admission although it is getting progressively more difficult to gain admissions. There are varying perspectives on the relative importance of sections, wherein the PAT or reading comprehension can be viewed as the most important or conversely, ignored. The PAT in particular is most often viewed as a threshold score, and therefore is the only score not included in the academic average; the threshold varies between 16 and 18.

MFDS Membership of the Faculty of Dental Surgeons

MFDS is an exam offered by the Royal College of Surgeons. It stands for Membership of the Faculty of Dental Surgeons. The exam is a requirement to allow further specialist training for dentists in the UK and some Commonwealth countries. If a dentist has this exam it indicates that he or she has undertaken additional study over and above the minimum qualifying level.
It has traditionally been seen as a more difficult exam encompassing a greater breadth and depth of subject matter than the MFGDP (Membership of the Faculty of General Dental Practitioners.) The MFGDP aims to test the knowledge and competency of the general dental practitioner, whereas the MFDS tests more specialist knowledge.
In order to be eligible to sit the MFDS exam a dentist must also have worked in both general dental practice AND a hospital (usually maxillofacial surgery) or community dental post. For these reason MFDS rather than MFGDP has been used as the preferred entry requirement for all higher specialist training in dentistry in the UK.
The Royal College of Surgeons will soon be amalgamating these two exams in order to make specialist training more accessible to those dentists who do not want to work outside of general dental practice prior to higher training. It is not yet clear how this will affect the quality of candidate entering specialist dental training

1. Synopsis :

Following 13 years of utilization of the Pulpotec Filling Paste on approximately 3'000 permanent and temporary molars with a clinical success rate of 80%, it would appear that the advantages offered by its utilization are clearly superior to any inconveniences, which are only minor, under normal conditions of use.

2. Introduction :

It was following an apical leak during a technique of total pulpectomy carried out about 15 years ago on an inferior molar, which was developing in a worrying fashion, that the idea was born of updating pulpotomy. This had already been proposed by A. Marmasse, Professor at the Paris Dental School a few years previously.

The usual treatment applied to pulpitis on a living tooth involves removing the totality of the pulp in the treated tooth and filling the cavity with an obturating material like an antiseptic paste, based on Zinc Oxide and Eugenol, or a paste based on Bakelite or condensed Gutta. This type of operation, when practiced on a molar, provises to be long and complicated due to the complexity of the shape of the roots, the narrowness of the buccal cavity and the difficulty of inducing total anesthesia in mandibular molars in particular.

In this sense, recent studies (1) have shown that the failure rates of endodontic treatments by pulpectomy are over 50% throughout the world, including Switzerland, Sweden and the U.S.A. These failures are characterized by an infection which starts at the extremity of the root. One must then imperatively have access to the roots of the tooth in order to repeat the treatment. This is not always easy due to the fact that any paste for canals, in particular Bakelite, is particularly difficult to remove. If the tooth cannot be treated again, it will have to be extracted. In addition, studies concerning the anatomy of the radicular canals (2) have shown that these contain numerous branches which cannot be reached during a total pulpectomy.

In addition, the practice of pulpectomy is accompanied by numerous risks like the inhaling by the patient of canal instruments used to extract the nerve from its cavity or the penetration of obturating material beneath the roots. In this latter case, the complications can be extremely serious, since they range from fungal sinusitis of the upper maxillary (3) to the onset of permanent pain in the lower maxillary for which no therapy currently exists (4-5).

At one time, the practice of pulpotomy was considered to be an alternative to pulpectomy. Rather than remove the totality of the pulp in a tooth, the dental surgeon performed a selective amputation in the coronal part of the tooth pulp contained in the pulpal chamber, and then sealed the cavity with a classical sealing material.

Amongst these, one can mention significant time saving, easy access to the part of the pulp which is to be treated and simplified technical procedures for the practitioner. The combination of these factors means that the principal risks which accompany pulpectomy are eliminated, in particular those linked with the use of canal instruments and to the leakage of filling material beneath the roots.

Nevertheless, the obturating materials used to date for pulpotomy have not been satisfactory, as they have caused cases of degeneration and infection in the residual pulp. Consequently, due to the numerous difficulties based on the inadequacy of the obturating material used, the practice of pulpotomy has been abandoned, despite its indisputable advantages. It should also be noted that pulpotomy is the only technique which authorizes complete radicular edificaton of immature permanent molars.

Recent studies (6), taking into account the observations of Professor A. Marmasse on pulpotomy (7), have revealed that the pulp has a great capacity for healing. This has led us to investigate the possibilities of perfecting a filling material capable of compensating for the inconveniences mentioned above and also generating the cicatrisation of the radicular pulp and its perenniality with the help of substances which would initially ensure antisepsis and hemostasis and then the maintenance of long term antisepsis and impermeability by means of a good volumetric stability.

دراسات حديثة (6)، آخذين بالحساب ملاحظات البروفسور مارماز على بتراللب (7)، التي أظهرت أن اللب لديه قدرة كبيرة للشفاء. هذا أرشدنا إلى أن نبحث في احتمالات اتقان مادة حشي لا عيب فيها قادرة على أن تعوض عن العقبات المذكورة فوق وأيضاً إنتاج تندب اللب الجذري ودوامه بمساعدة مواد يمكنها أن تضمن في البداية التطهير ووقف النزف وبعد ذلك المحافظة على تطهير لمدة طويلة والكتامة بمنع نفوذ السوائل وذلك بواسطة ثبوتية الحجم الجيدة.

3. Treatment:

In 1989 it was decided (after having treated a few cases in 1987) to treat all the permanent and temporary molars of the patients exhibiting pulpitis on a vital tooth by pulpotomy, being convinced that the risks incurred were much less important than those linked to a pulpectomy, a failure implying, in the majority of the cases, a simple return to treatment by so -called total pulpectomy.

المعالجة:

في 1989 قرر (بعد معالجة بعض الحالات في 1987) معالجة كل أرحاء المرضى الدائمة والمؤقتة الحية التي تظهر التهاباً لبياً ببتراللب مقتنعين أن المخاطر التي تحدث هي أقل أهمية من تلك التي تتعلق باستئصال اللب، فشل متوقع، في غالبية الحالات، العودة ببساطة إلى المعالجة المسماة باستئصال كامل للب.

Pulpotomy consists in amputating the cameral pulp and then capping the pulpal stumps with an obturating paste. To do this, the ceiling of the pulpal chamber should be removed by means of a Zekrya surgical bur, then the cameral pulp should be removed with a Zekrya Endo type bur. The pulpal chamber should be re-shaped with a pear-shaped diamond bur. It is important to use high speed rotary instruments in order to avoid tearing the radicular filaments, which must imperatively remain in the canals.

بتراللب يتضمن بتر اللب في الحجرة اللبية ثم تغطية اللب المتبقي في الأقنية الجذرية بعجينة سادة. للقيام بهذا، يجب إزالة سقف حجرة اللب باستخدام سنبلة جراحية (زكريا) , بعد ذلك يجب إزالة اللب في الحجرة اللبية باستخدام سنبلة زكريا اللبية . يجب إعادة تشكيل حجرة اللب بواسطة سنبلة ماسية بشكل اجاصة. من الضروري استعمال أداة دوارة ذات سرعة عالية من أجل تجنب تمزق الخيوط (الشعيرات) الجذرية، والتي يجب بشكل حتمي أن تبقى في القنوات.

The pulpotomy as such is completed by placing Pulpotec in the cavity chamber by means of a rotary Paste Filler of large diameter. The Pulpotec is then covered with a temporary dressing. A cotton roll should then be inserted between the two arches and the patient requested to bite firmly so that the Pulpotec adheres closely to the walls of the pulpal chamber and the openings of the radicular canals, thereby assuring perfect impermeability.

تتم عملية البتر عند وضع البلبوتيك في حجرة الحفرة بواسطة بوربات لحشي العجينة ذات قطر كبير. ثم يغطى البلبوتيك بضمادة مؤقتة. بعد هذا يجب وضع لفافة قطنية بين القوسين ثم يطلب من المريض أن يعض بصلابة (ثبات) من أجل أن تلتصق عجينة البلبوتيك بجدران حجرة اللب وبفوهات (فتحات) قناة الجذر، ذلك يؤمن عدم نفاذ السوائل بشكل ممتاز.

During a second session, approximately 8 days later, a permanent filling should be placed and, where necessary, the corresponding prosthesis.

خلال الجلسة الثانية، تقريباً بعد 8 أيام، يجب وضع عجينة حشي دائمة وعند الحاجة التعويضات الموافقة .

4. Clinical investigation:

To date approximately 3'000 teeth have been treated, half of which were permanent and the other half temporary.

An initial X-ray was taken on the day of treatment by pulpotomy and then, after the first 3 years, each time the patient returned to the practice. (The X-ray follow-up was carried out for permanent teeth only).

To this day, 310 radiological cases have been followed for periods between 3 and 13 years (3 years : 48 teeth; 4 years : 54 teeth; 5 years : 50 teeth; 6 years : 26 teeth; 7 years : 20 teeth; 8 years : 26 teeth; 9 years : 36 teeth; 10 years : 21 teeth; 11 years : 17 teeth; 12 years : 8 teeth; 13 years : 6 teeth; 15 years : 1 tooth). When analyzing the results, the absence of pain for a long period was also taken into consideration as well as the efficiency of mastication.

حتى اليوم، 310 حالة توبعت شعاعياً لمدة ما بين 3 إلى 13 سنة (3 سنوات: 48 سن، 4 سنين: 54 سن، 5 سنوات: 50 سن، 6 سنوات: 26 سن، 7 سنوات: 20 سن، 8 سنوات: 26 سن، 9 سنوات: 36 سن، 10 سنوات: 21 سن، 11 سنة: 17 سن، 12 سنة: 8 أسنان، 13 سنة: 6 أسنان، 15 سنة: سن واحد). عند تحليل هذه النتائج، غياب الألم لمدة طويلة قدأخذ بعين الاعتبار بالإضافة إلى فعالية المضغ .

In the majority of cases concerning permanent teeth, a molded prosthesis was placed in order to ensure the best possible tightness of the filling. In addition, it should be noted that this prosthesis also acts as an “early-warning device” because, given its cost, the patient will not hesitate to come for a consultation in case of pain and pain could indicate an infection of the radicular system.

5. Results :

5.1) Deciduous teeth :

Immediate pain relief after treatment in 90% of cases; mild pain on which lasts 2 to 3 days in 20% of cases, which persists for 2 to 3 days and stops afterwards. These teeth are clinically mute and function until they disappear from the dental arch to give way to the permanent teeth which follow them. This is the case for deciduous teeth which have remained in place until their "natural" elimination. As these teeth had not been crowned, approximately 30%, fractured during mastication and generated a leakage of Pulpotec. This caused an infection of the tooth and its extraction by the practitioner.

5. النتائج:

5-1) الأسنان اللبنية :

عدم شعور بالألم فوري بعد المعالجة عند 90% من الحالات: ألم خفيف يمتد حتى يومين أو ثلاثة عند 20% من الحالات، يستمر هذا الألم حتى يومين أو ثلاثة ثم يتوقف بعد ذلك. هذه الأسنان صامتة سريرياً وتعمل حتى تختفي من القوس السني لتمنح مكان للأسنان الدائمة التي تظهر بعدها. هذه هي الحالة للأسنان المتساقطة التي بقيت في مكانها حتى سقوطها "الطبيعي". بما أن هذا السن لم يتوج، تقريباً 30% تنكسر خلال المضغ وتسبب تسرب للبلبوتيك. هذا يسبب تلوث (التهاب) السن واقتلاعه من قبل الطبيب الممارس

5.2) Permanent teeth :

a) The 310 cases X-rayed present a healthy physiological image with no trace of periapical infection in the second X-ray, added to which, these teeth were not painful and mastication was perfectly normal.

5-2) الأسنان الدائمة:

أ) الحالات 310 المصورة شعاعياً أظهرت صورة فيزيولوجية صحية (سليمة) من دون أي أثر تلوث (التهاب) حول ذروي في ااتصوير الشعاعي الثاني، إضافة إلى هذا، هذه الأسنان لم تكن مؤلمة والمضغ كان طبيعياً بشكل تام.

During the same period of 13 years, 61 permanent teeth had to undergo further treatment by total pulpectomy. It seems logical to be able to compare these two figures. Effectively, in both cases, it is impossible to follow all the patients treated. This suggests a success rate of 80%, a figure which also corresponds to that quoted by Professor A. Marmasse in his work (7).

خلال نفس المدة 13 عاماً، 61 سن دائم مر بمعالجة إضافية باستئصال كامل للب. يبدو من المنطقي أن تكون قادراً على مقارنة هذين الرقمين. بشكل فعال، في كلتا الحالتين، يكون من الصعب متابعة كل المرضى الذين عولجوا. هذا يبين أن نسبة النجاح هي 80% هذا الرقم يتوافق مع الرقم الذي حدده البروفسور أ.مارماز في عمله (7).

b) Undesirable side-effects :

Following utilization of Pulpotec, 3 types of undesirable side-effects have been noticed :

* In 15% of the cases of teeth treated, a more or less intense pain of arthritic type was felt during the days which followed the treatment. It lessened progressively on its own or with the help of anti-inflammatory medication taken orally to speed up the process.

لدى متابعة استعمال البلبوتيك، لوحظ 3 أنماط للآثار الجانبية الغير مرغوبة:

- في 15% من حالات الأسنان المعالجة، شعر تقريباً بألم قوي من النوع الرباطي خلال الأيام التي تتبع المعالجة. لقد خف الألم تدريجياً بمفرده أو بمساعدة دواء مضاد التهاب أخذ مبكراً ليسرع العملية.

* In 3% of the cases of teeth treated, pain of medium intensity was felt until the second session 8 days later (also quoted by Professor Marmasse). This was treated by the immediate removal of the Pulpotec from the pulpal chamber, followed by the placement of a new dose of Pulpotec. Pain ceased immediately in each case.

- في 3% من حالات الأسنان المعالجة، ألم من الشدة المتوسطة شعر به حتى الجلسة الثانية بعد 8 أيام (حدده أيضاُ البروفسور أ.مارماز في عمله). لقد عولج هذا بإزالة فورية لكل آثار البلبوتيك من حجرة اللب يتبع هذا استعمال جرعة جديدة من المنتج. يتوقف الألم مباشرة في كل حالة.

* Exceptionally (2 cases in this study) pain of high intensity can occur. This was due to the fracture of a tooth which was not detected during the initial examination or to the perforation of the floor of the pulpal chamber which had also been overlooked. In such cases the tooth most often has to be extracted which is precisely the same consequence which would have prevailed following treatment by pulpectomy.

- باستثناء (حالاتان في هذه الدراسة) يمكن أن يظهر ألم ذو شدة حادة. هذا بسبب كسر في السن لم يشخص خلال الفحص المبدئي أو بسبب ثقب قاع حجرة اللب والتي أيضاً قد غفل فحصها. في هذه الحالات غالباً يجب قلع السن هذه تماماً نفس النتيجة التي ستطبق بعد المعالجة باستئصال اللب الكامل .

6. Discussion and conclusions :

Obviously, it is difficult to expect that X-rays and a clinical follow-up alone will prove the perenniality of pulpal vitality after treatment with Pulpotec.

منقاشة ونتائج:

من الواضح، أنه من الصعب أن نتوقع من الأشعة السنية والمتابعة السريرية وحدها أن تثبت تعمير (دوام) حيوية اللب بعد المعالجة بالبلوتيك.

Proof of this type, stated by Dr Marmasse to be very difficult to establish after vital pulpotomy (7), has not been included in these clinical trials for both ethical and practical reasons.

اثبات هذا النوع، أعلن الدكتور مارماز من الصعب تحدديه بعد بتر لب حي (7)، لم يشمل في هذه التجارب السريرية لأسباب أخلاقية وعملية.

On the other hand, post-operative functional tests, as well as the X-rays, appear to confirm the perenniality of pulpal vitality after treatment with Pulpotec.

ومن جهة أخرى، الاختبارات الوظيفية للأسنان بعد العلاج الإضافة إلى الأشعة السنية، تبدو أنها تثبت دوام حيوية اللب بعد المعالجة بالبلوتيك.

This is further confirmed by the fact that the follow-up of immature permanent molars treated with Pulpotec, has indicated normal, progressive radicular edification until complete maturity.

ويعزز اثبات هذا أيضاً بحقيقة أن متابعة الأضراس الدائمة غير الناضجة المعالجة بالبلوتيك، دلت على تعديل ( تشكل ) جذري طبيعي ومتقدم حتى نضوجه الكامل.

Finally, if one considers that, on the one hand, in addition to the important risks linked to periapex, the further risks linked to pulpectomy are (with the exception of point N° 2) identical to those linked to pulpotomy, that is to say essentially pain of an arthritic type and, on the other hand, the percentage of clinical failure is significantly higher for pulpectomy than for pulpotomy (50% versus 20%), it would seem sensible to prefer this technique. All the more so as it grants an additional span of life for the tooth and simplifies the work of the dental surgeon. As a result, it can be considered more reliable and likely to optimize the chances of survival for the teeth treated by pulpotomy with Pulpotec.

أخيراً, إذا أمعن فكر الشخص بهذا، من ناحية، بالإضافة إلى المخاطر المهمة المتعلقة بحول الذرة ، المخاطر الإضافية المتعلقة باستئصال اللب هي (باستثناء نقطة رقم 2) مماثلة لتلك المتعلقة ببتر اللب، يعني ألم أساسي من النوع الرباطي ومن ناحية اخرى نسبة القشل السريري أكبر بشكل كبير عند استئصال اللب من بتر اللب (50% مقابل 20%)، يبدو من المعقول تفضيل هذه التقنية. بالإضافة إلى كل ذلك هذه التقنية تقدم مدة حياة إضافية للسن وتسهل عمل طبيب الأسنان الجراح. نتيجة لذلك، يمكن الاعتماد عليها أكثر ومن المحتمل أن تقدم فرص بقاء السن المعالج ببتر اللب بالبلوبتيك حياً.
